Photograph and email publication release

I hereby authorize Midwest Recruiting, LLC use of my child's likeness, image, photograph and video as supplied by me. The use includes but may not be limited to the Recruit Profile page that I am purchasing. Additionally, I authorize the publication of my child's email address on the Recruit Profile page as a contact reference for college coaches.
